方法: 全面检索Pubmed、Excerpta Medica Database (Embase)、Chinese Biomedical Literature Database (CBM)、the Cochrane Library、维普及万方数据库, 搜集关于miRNA-146a rs2910164单核苷酸多态性与肝癌易感性相关性的研究. 对miRNA-146a rs2910164 G/C各基因型比较模型(G vs C、GG vs CC、GG vs GC、GC vs CC、GG+GC vs CC、GG vs GC+CC)在病例组及对照组的分布情况进行定量综合分析.
结果: 共纳入8篇文献, 共有肝癌患者及健康对照5968例. Meta分析结果显示, miRNA-146a rs2910164单核苷酸多态性与肝癌易感性之间具有相关性(GC vs CC: OR = 1.15; 95%CI: 1.02-1.92, P = 0.02; GG+GC vs CC: OR = 1.15; 95%CI: 1.03-1.29, P = 0.01). 亚组分析发现, 在亚洲人群中也有相似的结论(GC vs CC:OR= 1.15, 95%CI = 1.02-1.29, P = 0.02).
Association between rs2910164, a single nucleotide polymorphism in miRNA-146a, and susceptibility to hepatocellular carcinoma: A Meta-analysis

AIM: To accurately evaluate the association between the miRNA-146a rs2910164 polymorphism and susceptibility to hepatocellular carcinoma by Meta-analysis.
METHODS: An electronic search of PubMed, Excerpta Medica Database (Embase), Chinese Biomedical Literature Database (CBM), the Cochrane Library, Weipu and Wanfang database for relevant articles was conducted. The difference of miRNA-146a rs2910164 genotypes (G vs C, GG vs CC, GG vs GC, GC vs CC, GG + GC vs CC, and GG vs GC + CC) between cases and controls was analyzed through Meta-analysis.
RESULTS: A total of 8 studies involving 5968 subjects were found to be eligible for our meta-analysis. There was evidence of association between miR-146a rs2910164 polymorphism and the risk of hepatocellular carcinoma in the overall analysis (GC vs CC: OR = 1.15; 95%CI: 1.02-1.92, P = 0.02; GG + GC vs CC: OR = 1.15; 95%CI: 1.03-1.29, P = 0.01). In the subgroup analysis by ethnicity, similar results could be found among Asians (GC vs CC: OR = 1.15, 95%CI = 1.02-1.29, P = 0.02).
CONCLUSION: Our Meta-analysis suggested that the rs2910164 polymorphism in miR-146a was more likely to be associated with hepatocellular carcinoma risk. Besides, the CC genotype of rs2910164 might be associated with a reduced risk for hepatocellular carcinoma.

对纳入的研究进行定量综合分析, 各基因比较模型的合并OR及其95%CI结果如表2所示: 在总体分析中, miRNA-146a rs2910164单核苷酸多态性与肝癌易感性之间具有相关性(GC vs CC: OR = 1.15; 95%CI: 1.02-1.92, P = 0.02; GG+GC vs CC: OR = 1.15; 95%CI: 1.03-1.29, P = 0.01). 亚组分析发现, 在亚洲人群中也有相似的结论(GC vs CC: OR = 1.15, 95%CI = 1.02-1.29, P = 0.02). 为评估纳入文献的发表偏倚情况, 做了Begg's漏斗图(未将漏斗图纳入本研究)及Egger's线性回归分析(表3), miR-146a rs2910164 G/C各基因模型的Begg's漏斗图基本对称, 其Egger's线性回归分析结果的P值
均大于0.05, 表明不存在发表偏倚. 为评估研究结果的可靠性, 对本次Meta分析进行敏感性分析. 结果发现, 本次Meta分析结果不稳定: 在GC vs CC的比较模型中, Huang、Li及Xu等的研究对合并效应量有较大影响, 依次剔除Huang、Li及Xu等的研究后, 合并OR值由1.15(95%CI: 1.02-1.29, P = 0.02)分别变为1.12(95%CI: 0.99-1.27, P = 0.07)、1.09(95%CI: 0.96-1.25, P =0.18)及1.13 (95%CI: 0.99-1.29, P = 0.06); 而在GG+GC vs CC的比较模型中, 依次剔除Li及Xu等的研究后,合并OR值由1.15(95%CI: 1.03-1.29, P = 0.01)分别变为1.09(95%CI: 0.96-1.23, P = 0.18)、1.12(95%CI: 0.99-1.27, P = 0.07).
3 讨论
目前, 有多个报道研究了miRNA-146a rs2910164单核苷酸多态性与肝癌易感性之间的相关性, 包括原始研究及相关Meta分析. 但是, 这些研究间的结果不尽一致, 且Meta分析纳入的文献数量有限. 为此, 本文在全面搜集相关文献的基础上, 对miR-146a rs2910164单核苷酸多态性与肝癌易感性之间的相关性进行系统而全面的评价. 研究发现, 在总体分析中, miRNA-146a rs2910164单核苷酸多态性与肝癌易感性之间具有相关性(GC vs CC: OR = 1.15; 95%CI: 1.02-1.92, P = 0.02; GG+GC vs CC: OR = 1.15; 95%CI: 1.03-1.29, P = 0.01). 亚组分析发现, 在亚洲人群中也有相似的结论. 分析结果发现, miRNA-146a rs2910164单核苷酸多态性与肝癌易感性之间具有相关性, 并且miRNA-146a rs2910164单核苷酸多态性的CC基因型可能是肝癌的保护因素. 发表偏倚检查中发现, 本研究纳入的文献不存在发表偏倚. 敏感性分析发现, 本次Meta分析结果不稳定.
