自从1971年Folkman et al [1]首次提出肿瘤的生长和转移具有血管依赖性以后, 肿瘤血管生长的调节机制和抗肿瘤血管生长在肿瘤治疗中的意义得到了广泛关注和深入研究. 由于难以获得纯化的血管抑制剂, 早期研究进展缓慢. 1994年O'Reilly et al [2]从接种Lewis肺癌细胞株的小鼠尿中发现并提纯的第1种肿瘤血管抑制剂-血管抑素(angiostatin), 使抗肿瘤血管生长的研究突飞猛进, 且有望成为第1种应用于临床的肿瘤血管抑制剂.
肿瘤血管生长的调控机制, 目前多数学者[11-28]接受Hanahan et al [29]提出的血管调控平衡学说(angiogenic switch). 认为肿瘤细胞、内皮细胞和巨噬细胞受缺氧、系统刺激、基底膜通道的信息传递等使局部微环境发生变化的因素作用后出现一系列基因表型变化, 打破原先的促血管生长因子(血管内皮生长因子、碱性成纤维细胞生长因子和血小板源性内皮细胞生长因子等)和血管生长抑制因子(血管抑素、内皮稳定素和β转化生长因子等)之间的平衡状态. 当促血管生长因子的作用强于血管生长抑制因子时, 便引起肿瘤血管生长.
