¹Screening colonoscopy indications defined as follows: (1) Patient: colonoscopy performed in individuals without a history of either gastrointestinal conditions in the past 12 mo or large bowel symptoms in the past 6 mo; (2) Endoscopist^a: colonoscopy performed in asymptomatic individuals or those with a family history of colorectal cancer; and (3) Endocopist^b: colonoscopy performed in asymptomatic individuals or those with a family history of colorectal cancer; or reported as confirmatory colonoscopy (performed to follow-up on a positive screen).
