A: A stem from an ipsilateral total knee arthroplasty prevented revision with a long extensively porous coated implant. A stage 1 revision was performed with retention of the pedestal to prevent cross-contamination followed by 6 wk of IV antibiotics with subsequent femoral impaction grafting; B: With subsequent femoral impaction grafting; C: 63 year-old female 3 mo after femoral impaction grafting. Allograft struts were applied to bypass a stress riser distally, and metal mesh was used to reconstruct the calcar.
