³Smoking history, at least one cigarette per day for 6 mo or longer;

⁴Alcohol drinking history, at least twice a week for 6 mo or longer and at least 100 g each time;

⁵Family history of gastric cancer, first-degree relatives with gastric cancer (parents, brothers or sisters). FH: Family history of gastric cancer.
